Depending on the nature of the situation, students may be seen at NC State's Student Health Services.

Call them first before going to their center to confirm they will treat the injury.

For minor injuries, the supervisor should complete the top portion of the Supervisor's Medical Treatment Authorization Form and provide it to the employee before they go to urgent care.

  • Call 911 immediately!
    The operator will advise whether to take the individual to urgent care, the emergency room or wait for an EMS.
  • Alert Environmental Health and Safety (919-515-5445) and the individual's supervisor or mentor immediately.

  • Amputation or Loss of Body Part
  • Blood or Body Fluid Exposure
  • Chest or Abdominal Injuries
  • Compression Injuries
  • Head Injuries
  • Injuries contaminated with chemical, biological or radioactive material
  • Non-Clean Sharp Objects Injuries (e.g., needle sticks)
  • Slurred Speech Caused By Injury
  • Seizures Caused By Injury
  • Severe Altered Level of Conscious
  • Severe Burns
  • Severe Falls
  • Severe Trauma (e.g., broken bones, fractures)
  • Spinal Injuries
