Accidents Requiring Medical Attention

• Dial 911 from a campus phone (or 919-515-3000 from a cell phone) to contact campus police to determine appropriate transportation to the urgent care facility.
• Take a Medical Treatment Authorization form with you to one of the urgent care facilities below.

*Very minor injuries may be able to be treated at Student Health during business hours (but call first!).

Accident Forms

​Once the injury has been treated, complete the following forms.

• Supervisor’s First Report of Incident
• Workers’ Compensation – Employee Statement
• Workers’ Compensation – Use of Leave Options

After the forms have been completed, email all in a PDF format to the following email addresses.
